Advance Care Planning is the process of thinking about and sharing your wishes for future medical care in the case that an accident or serious illness leaves you unable to talk to your loved ones and medical team. This includes making your wishes known in a legal document called an advance directive. In your advance directive, you are able to designate a patient advocate. A patient advocate is a trusted person who you select to follow through on your wishes and make medical decisions if you are unable to.
