Release of Medical Records Patient Guide
The goal of this Patient Guide is to tell patients how to get their medical records. Medical records include test results and x-rays. McLaren Greater Lansing must make sure that the medical records are given out correctly. McLaren Greater Lansing takes their role very seriously and works hard to protect patient privacy. Please note:
- Patients do not "own" their medical records, they control access to it.
- The patient must ask for medical records in writing.
- The form used to ask for medical records is an Authorization, but can also be called a Release.
- Only some Departments give out copies of medical records.
- Any person picking up medical records will be asked to show picture ID.
- Copies of medical records cannot be given out right after a hospital stay or a test is finished.
- Copies will be sent to your doctor(s) when they ask.
- A copy or fax of a Release will be accepted.
- Per Michigan law, a Release is good for 60-days from the date signed.
HOW TO GET MEDICAL RECORDS
The Release is filled out. This will tell McLaren Greater Lansing who can have medical records about the patient. A blank Release is attached. Extra copies are available in the Medical Records Department, Radiology Department, Emergency Department, Patient Accounts Department, and inpatient floors. Extra copies can also be mailed or faxed to the patient, or are available at www.mclaren.org/lansing.
The Release is given to McLaren Greater Lansing. The form can be dropped off, mailed or faxed. If the form is not complete, the form may be returned.
McLaren Greater Lansing will check the form. If the form is valid the information will be copied. Copies can be mailed or picked up. McLaren Greater Lansing does not fax records except when your doctor asks.
NOTE: Due to the number of requests for copies, requests are done in the order received.
The Lab does not give copies of medical records to patients. Copies of results from tests come from Medical Records. At least 24-hours is needed before results are ready.
MEDICAL RECORDS DEPARTMENT
Gives copies of information about patient care at McLaren Greater Lansing, including test results from Lab and X-Ray (if the films are not needed). McLaren Greater Lansing does charge for copies. Payment can be made by credit/debit card, check or money order to "HMSI." If a patient can show that they cannot pay, one copy will be given for free. Speak to staff with any questions.

Gives test slides.

Gives copies of x-rays for follow-up for doctor(s) appointments. X-Rays can be given in film or CD form.Unless requested, x-rays will be on a CD. McLaren Greater Lansing may charge for copies. Payment can be made in cash, or by check or money order to "McLaren Greater Lansing." At least 24-hours is needed for x-rays to be ready.

|Medical Records is on the 1st Floor of the Main Hospital (behind the Pro Building on the map). There is parking in the lot of the Pro Building. Handicap parking with access to this part of the Main Hospital is in the lower level of the Parking Ramp.
Radiology is on the 1st Floor of the Chi Heart & Surgery Center. Parking is in the main parking lot to the East of the Chi Heart & Surgery Center.
Pathology is on the 4th Floor of the Main Hospital. Use the Elevator near the Chi Library. (Best access is through the entrance behind the Pro Building on the map.)
