We participate with most local and many national insurance plans. However it is your responsibility to understand your coverage and whether or not your insurance has limits on the doctors you can see or the services you can receive as a covered benefit.  If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier and receive direct payment for covered services on your behalf. 

Office Visit Copayments are expected at the time of your appointment.  When possible, co-insurance and deductible obligations as well as charges for non-covered services will be estimated for payment or partial payment at time of service.  Balances and amounts not estimated, along with any other charges, are billed via statement.  Patient statements are mailed monthly and payment is due upon receipt of your statement.

MMP accepts payments in the form of check, money order, or credit card.

  • Please make check or money order payments payable to Mid-Michigan Physicians, PC and include the patient account number on your check.                                                        

Cash payments can be made in person at your physician’s clinical office or at the Central Billing Office.

Payment Plans:
Payment plans are available to assist patients with large balances. If full payment cannot be made at the time of service or at the time of the 1st statement, arrangement of a payment plan will provide opportunity for patients to pay their account balance off over time. Terms will be set up so that payment in full is in place within a 5 month period.  If you have a significant balance and are unable to pay your balance under the 5 month plan you may apply for individual consideration of your personal circumstances and resources that could extend payment terms up to a 12 month plan.

Failure to make payment arrangements or payment of any copayment, co-insurance, deductible, or non-covered amounts could result in a referral of patient accounts to an outside collection agency.  If your account is referred for collections you will not be allowed to schedule future appointments for elective or non-urgent medical care until your account is paid in full.

Billing for medical services related to Auto Accidents

  • If you are ever injured as the result of a motor vehicle accident, you will be responsible for filing an accident claim with your auto insurance company. Because Michigan is a “No Fault” state, residents are each covered by their own auto insurance, without regard for who may be liable in an accident. When receiving care for an injury related to a motor vehicle accident, you will need to provide MMP with your auto insurance information as well as your health insurance information.  We will submit an itemized claim on your behalf coordinating with your health insurance as may be appropriate based on your policy and coverage.  Any non-covered charges are your responsibility.

Workers’ Compensation

  • For patients injured at work, claims are sent directly to their employer or their employer's Worker's Compensation carrier. It is the patient's responsibility to make sure their employer completes both an accident claim and the appropriate Worker's Compensation forms.  If Workers’ Compensation does not cover care, the group health plan may be billed on the patient’s behalf.  If the Workers’ Compensation claim is disputed, the patient must notify MMP promptly.  Any non-covered charges are your responsibility.

Patient services provided during the following hours of operation:

  • Monday through Thursday 8:00a.m. - 4:30p.m. and Fridays 8:00a.m. - 2:30p.m. Extended hours offered at some locations.

Some of our clinical offices offer extended morning hours and/or extended evening hours.

Many of our patients wish to see a health care provider during the evening or on the weekend outside of normal business hours.  This option is provided as a customer service choice and may be a convenient benefit to some, allowing the opportunity to obtain medical care while not missing work or during times where child care is available or while conflict with other personal time obligations are minimized.

Most insurers recognize the extra costs associated with extend hours and recognize this in their benefit structure and have agreed to supplement their usual fee schedule and allowed amounts in these circumstances.  Any questions or concerns about coverage for this service should be placed with your insurer’s Customer Service line to ask about your coverage for this service.

For our patients who choose to be seen during extended hours (before 8:00 a.m. or after 5:00 p.m. Monday through Friday) MMP adds a $23.00 surcharge to our fees in compliance with Blue Cross/Blue Shield and other insurers’ billing guidelines.  (The CPT code is 99051.)  Patients are required to pay all copay, co-insurance, or deductible amounts applied to this after-hours fee by their insurer.
