The Hospice Team

McLaren Hospice is an interdisciplinary team of clinical professionals with a special expertise in pain and symptom management, enabling them to meet the unique needs of each hospice patient. Our team members include:the hospice team graphic

Medical Director — advisor to the hospice team and available 24 hours a day, seven days a week.

Registered Nurses — visit the patient’s home regularly to provide nursing care and instructions to family members with regard to medications and the skills needed to care for the patient.

Home Health Aides — provide personal patient care (i.e. assist with dressing, bathing and eating).

Medical Social Workers — help the patient and family cope with the many adjustments an illness sometimes creates in the family unit.

Spiritual Care Coordinators — serve as the liaison between the patient/family and their minister, priest, rabbi or spiritual counselor of their choice.

Volunteers — specially trained individuals who provide non-medical support and assistance to patients and families such as respite care, companionship and help with errands.

Registered Dietitian — consults with hospice team members, the patient and the family concerning the nutritional needs and problems that the patient may be experiencing.

Bereavement Coordinator — arranges bereavement services including reassurance, support and education to surviving family and friends. Bereavement services continue for 13 months after the death of the patient.
