PCMH Information

Patient Centered Medical Home (PCMH) is a healthcare setting that facilitates a philosophy of a strong patient-physician relationship. It includes an agreement from both the patient to be actively engaged in their health and the physician to work together with the patient, and when appropriate their family, to provide comprehensive healthcare with all parties working toward achieving wellness. The professional organizations adopting and supporting this model of care include the American Academy of Family Practice, American College of Physicians, American Osteopathic Association, and the American Academy of Pediatrics.

Patient Centered Medical Home Neighborhood (PCMH-N):
An extension of patient centered medical home into specialists and sub-specialists fields which includes a strong PCP / Specialist relationship to ensure communication and improved outcomes of mutual patients.
