VitalCare brings clinical care and support services into the home for patients who are transitioning from a medical facility after stroke, illness, or surgery, and for patients who are living with a chronic condition such as heart disease, chronic obstructive pulmonary disease, or diabetes. The VitalCare professional team promotes health and wellness for recuperating patients and improves quality of life for clients who are at-risk for repeated hospitalizations.

Medicare, Medicaid, Blue Cross, and many other insurers provide a home health benefit for their members. VitalCare is Medicare-certified and approved by all major insurance plans. Anyone can make a home health care referral. Every insurance plan has specific eligibility requirements for care. Our friendly Referral Center staff will contact your insurance company.

Private Duty Services

VitalCare Private Duty Services are tailored to meet the health needs and lifestyle requirements of individual patients and their families with professional care, three to 24 hours a day. Home care assistance is ideal for those with chronic, short- or long-term, or around-the-clock needs.
Family members also benefit from Private Duty Services with respite care on days when family members are not available or seek personal time. Communication with family members, care providers and physicians, and community agencies contribute to ease of care and overall quality of life.
