Patient-Centered Care

Patient-Centered Care is More than a Goal - it's a Reality
You and your well-being are the reasons we exist. Your needs are our priority, and your uniqueness will be recognized, respected, and appreciated in all that we do.

Patient Centered Care

Patients actively engaged in their health care report better outcomes. Patient-centered care involves not only including patients and their families in decision-making, but also means education, counseling, guidance through the course of treatment, and assistance with coordinating care at home.

Education and Wellness
From health education to wellness classes and events, the John and Marnie Demmer Wellness Pavilion and Dialysis Center promotes knowledge and exercise in a specially-designed facility, with state-of-the-art equipment including an exercise pool.

Primary Care Provider Network
McLaren Northern Michigan is expanding its network of primary care providers - the family medicine physicians that often develop life-long relationships with patients and their families - in order to provide a full and integrated continuum of services.

Oncology Nurse Navigator Assistance
Newly diagnosed cancer patients find themselves in a new and complex world of terminology, testing, and treatment. Oncology Nurse Navigators at McLaren Northern Michigan assist patients every step of the way, making sure patients have the support and information needed to stay committed to their fight against cancer. Nurse Navigators help patients find the right health care professionals for their needs, help patients make sense of diagnosis and pathology reports, coordinate appointments, and act as a resource for patients and loved ones throughout the treatment journey.

Care Coordination: From Hospital to Home
Care Coordinators at McLaren Northern Michigan not only work to ensure that patients understand their treatment while in the hospital, they also work to ensure that appropriate ongoing care is available for patients after discharge. Care Coordinators are skilled at matching services and information to patient needs. From outpatient rehabilitation at the John and Marnie Demmer Wellness Pavilion, to ongoing home care, private duty nursing, home medical equipment, or telehealth services available through VitalCare, the McLaren Northern Michigan health system provides a full continuum of care appropriate for individual patient needs.
