The St. Clair Community Health Center opened in February 2015.
The Lexington Community Health Center opened in May 1997.
The Marysville Community Health Center opened in January 1998.
The Yale Community Health Center opened in January 1999. Prior to its current location, it was located in the former Yale Community Hospital. It is located south of downtown Yale.
The Capac Community Health Center opened in downtown Capac in 2000.
