Persivia’s CareSpace TM will assist McLaren to achieve even greater success in our value-based care models. The platform combines care coordination, real-time quality and utilization analytics and reporting, enhanced patient and provider engagement, and bundle payment management tools. Here are some of the clinical functionality we will realize following implementation:

  • Automated Care Coordination work flows to improve efficiencies; allowing staff to spend more time managing patients
  • Risk stratification using artificial intelligence, to include Hierarchical Condition Category (HCC) codes
  • Post-acute care coordination and communication in a single, secure environment
  • Identifies and monitors bundled payment patients from the time of admission
  • Provides real-time monitoring and feedback on key clinical process and outcome metrics
  • The ability to engage patients and providers through integrated communication within the platform
  • Improved data aggregation (claims and clinical data) among electronic medical record systems
