Sometimes patients need a little extra care, and a little extra time, before they are ready to go home after a hospital stay. The Transitional Care Unit at McLaren Lapeer Region is an added level of nursing care that helps patients transition from hospital to home.

Conveniently located onsite at McLaren Lapeer Region, our Transitional Care Unit provides skilled nursing care and therapy services to residents who no longer require inpatient hospital care, but aren’t yet ready to return home. For many residents and families, it’s a welcome alternative to an off-site nursing home.

Residents in the 19-bed Transitional Care Unit have access to hospital-based care and specialty services, but the setting is much more relaxed and homelike. Our multidisciplinary care team works closely with each resident and family to develop a personalized treatment plan. Together, we help residents get back to performing their normal daily activities and return home.

Transitional Care Team

Residents in the Transitional Care Unit have a wide variety of needs – from medical care and nursing services to physical rehabilitation and social services. Our multidisciplinary care team works closely with each resident and family to develop a personalized treatment plan designed to address individual needs. Our care team includes:

Transitional Care Program

The Transitional Care Unit is a 19-bed skilled nursing facility with both private and semi-private rooms. Our location at McLaren Lapeer Region means hospital-based care and specialty services are just down the hall. This gives residents and families great peace of mind.

The Transitional Care Unit offers a comprehensive range of onsite services, including:

  • Physical Therapy – Therapists work with residents and their families to design specific treatment programs based on present functional levels and agreed-upon goals.
  • Speech Therapy – Speech-language pathologists evaluate and treat residents with voice, articulation and language disorders, as well as cognitive disorders and swallowing dysfunction.
  • Occupational Therapy – Occupational therapists evaluate residents and design individualized plans of care for residents who have difficulty performing everyday living activities.
  • Social Services – Our social worker helps residents with admission and discharge planning as well as referrals to support groups, educational programs and follow-up care.
  • Wound Management – When needed, wound care specialists are available to provide care and education to residents with ostomies and wounds.
  • Respiratory Therapy – Oxygen therapy and breathing treatments are available from McLaren Lapeer Region’s cardiopulmonary department.
  • Dietary Services – A registered dietitian evaluates the needs of each resident and makes appropriate dietary recommendations. Individualized diet education is provided based on resident needs.

Since the Transitional Care Unit is conveniently located at McLaren Lapeer Region, additional specialty services are available onsite, including:

  • Critical care
  • Emergency medicine
  • Diagnostic imaging
  • Lab services
  • Pharmacy

The Transitional Care Unit at McLaren Lapeer Region is a Medicare- and Medicaid-certified skilled nursing facility, which means services are often covered by Medicare and private medical insurance.

The Transitional Care Unit is designed to transition residents from hospital to home. The atmosphere here is more relaxed and homelike than a typical acute care hospital unit. Residents and families are encouraged to socialize and relax in the dining room. A big-screen TV and recreation activities are available in a comfortable family area. Snacks and refreshments are available in the nourishment center.
