Helping Your Patients Manage Diabetes

The McLaren Flint Diabetes Program makes the process easier with their comprehensive diabetes management program.

Patients have the power to take control of their diabetes.

Learning how to manage diabetes can be challenging for patients, but knowledge is empowering. McLaren Flint Diabetes Education is here to help guide your patients. Our team consists of a Registered Nurse and a Registered Dietitian, and our program has been accredited by the Association of Diabetes Care and Education Specialists.

We offer classes for type 1, type 2, and gestational diabetes.

Physicians may order diabetes classes, medical nutrition therapy, follow-up diabetes education, or our comprehensive diabetes management program, depending on your patient's individual needs.

What can your patient expect when you order diabetes classes?

Your patient's first visit will be with our registered nurse in person, via telehealth or by phone. This visit includes going over individual patient needs, goals, and planning of educational sessions.

The next step is a series of three classes that can be taken via telehealth or in person. Classes address meal planning, carbohydrate counting, blood sugar monitoring, exercise, medications, stress and coping, sick day management, and risk reduction. Classes are held in a setting where all are welcome and supported.

What can your patient expect when you order the comprehensive diabetes management program?

We will begin by addressing your patient's individual needs. Next, we will schedule educational classes with both our nurse and dietitian. Patients will also have up to six physician visits for diabetes medication adjustment to optimize their diabetes control.
