2020 PCP Value Based Reimbursement (VBR) &
2020-2022 Patient Centered Medical Home (PCMH) Designations
The BCBSM Value-Based Reimbursement (VBR) program offers additional revenue for initiatives such as: PCMH, Clinical Quality (CQI) performance, cost, and provider delivered care management (PDCM). This provides an opportunity to earn up to a 168% of the BCBSM fee schedule. As of 2020, a practice MUST be PCMH designated for the physician to achieve additional VBR opportunities.
We are excited to share our first 6 physicians who have achieved a PDCM VBR. This is based on an individual physician’s percentage of engaged patients with 2 outreaches for care management services on 2 different dates of service. The benefits of PDCM include improvement in patient outcomes, patient experience, and utilization across all settings. The VBR effective period is September 1, 2020– August 31, 2021.

7 out of 287 physicians achieved the NCQA benchmark at >95 percentile earning 115% CQI VBR. The VBR effective period is September 1, 2020 – August 31, 2021. Congratulations to our top-quality performers:

In addition, we are pleased to announce 109 out of 134 of MPP’s primary care practices achieved designation. Physicians attributed to these practice units achieved the PCMH VBR. This year starts a 2-year (2020-2022) PCMH designation period with an effective date of September 1, 2020 – August 31, 2022.
