For more than a 25 years, McLaren Home Care has cared for the residents of northern Michigan and helped them to avoid
unnecessary hospitalizations.

McLaren Home Care is accredited by the Community Health Accreditation Program (CHAP). In addition to successful patient outcomes and high customer satisfaction scores, McLaren Home Care has ranked in the top 25% of home health care organizations nationwide, based on Medicare reports. A separate, independent study ranked McLaren Home Care services at the 95th percentile among northern Michigan providers.

McLaren Home Care Nurses develop an individualized plan of care for each patient. In addition to communicating with your
physician, nurses provide education about your condition including medication dosage, as well as provide information to help you manage chronic illness.

Services include:

  • Infusion Therapy

  • Wound and Ostomy Care
  • Disease Management Education
  • Medication Education

Home health care nurses also schedule additional care and support.

Physical Therapists help you improve motor function and offer instruction about assistive devices and prosthetic care. They are often involved with patients who have suffered a stroke, had a joint replacement, or need to regain strength after illness or surgery.

Occupational Therapists enhance functional ability and recommend equipment and environmental adaptations.

Speech Language Therapists work with patients who have speech, communication, or swallowing disorders.

Medical Social Workers provide short term therapeutic counseling and make referrals to supportive community agencies.

Home Health Aides provide personal care and light housekeeping support.

Who Can Benefit
Home health care is beneficial following a hospital stay. A nurse will visit to assess your condition and provide additional clinical and supportive care during your recovery period.

Home health care is also beneficial for individuals who are managing a chronic health care condition at home such as heart disease, chronic obstructive pulmonary disease, or diabetes. Home health care nurses can teach families how to manage these conditions to avoid repeated hospitalization.

Medicare, Medicaid, Blue Cross, and many other insurers provide a home health benefit for their members. McLaren Home Care is Medicare-certified and approved by all major insurance plans. Anyone can make a home health care referral. Every insurance plan has specific eligibility requirements for care. Our friendly Referral Center staff will contact your insurance company.
