At Karmanos, our patients are our first priority and we try to schedule all new appointments within one week of the request.

Health History Questionnaire
Please complete this form before your appointment. You can fill it out online and submit it directly to our patient concierge, or print the form and complete prior to your appointment then bring with you to your first appointment. (A printed version is included with your mailed appointment confirmation packet.) When you arrive, let us know if you completed it online or have the completed from with you.
Authorization to Release Medical Information
This form is required for obtaining your medical records. We request that you fill out this form and fax it back to Karmanos Cancer Center as soon as possible in order for our patient concierge to begin collecting your medical records for our physicians to review prior to your appointment with us.
