Dear Provider/Pharmacist,
Thank you for your interest in providing services at the Helen M. Nickless Volunteer Clinic. The clinic provides primary care for uninsured and underinsured individuals who cannot afford health care. All of the health care provision is uncompensated and provided by licensed volunteer health care practitioners and pharmacists such as yourself.
You can volunteer as often as you like. Services are only offered on Wednesdays. Enclosed are the following forms:

Please return your application and confidentiality agreement, along with copies of your CV and licenses in the envelope provided. Upon receipt I will contact you to arrange dates.
