McLaren Hospice provides health care with an emphasis on quality of life at the end of life. Whether during a short-term hospital stay or wherever you call home, McLaren Hospice provides comfort, support and dignity to those with a life-limiting illness.

What is hospice care

Focused on the patient

Focused on treating the whole person, hospice provides medical care and emotional support for people facing the end of life. Hospice care also helps the family manage the practical details and emotional challenges of caring for a loved one nearing the end of life.  McLaren Hospice can provide care anywhere a patient calls home, including in an assisted living facility (ALF), skilled nursing facility (SNF), adult foster care home (AFC) or at the hospital.

When Hospice Is The Right Choice

Hospice Care is available for individuals in the McLaren service area who have a lifespan prognosis of six months or less.

Patients are accepted on the basis of need regardless of age, sex, religious affiliation, or nationality. Ability to pay is never a criterion for acceptance to Hospice Care.

McLaren Home Care & Hospice are nonprofit, Medicare-certified, Blue Cross-approved, and CHAP-accredited (Community Health Accreditation Program) providers of home healthcare programs and services.

Qualifications and coverage

Most insurance companies have a hospice benefit and McLaren Hospice will work directly with your insurance provider regarding coverage. Under Medicare Part A (hospital insurance) benefits, hospice coverage is provided for all services related to a life-limiting illness. Your loved one must meet the following criteria to be eligible for the Medicare Part A hospice benefit:

  • Your doctor and hospice medical director certify you have a terminal illness and are likely to live 6 months or less if your illness follows the normal course
  • You accept comfort care instead of seeking curative treatment for your illness

Hospice services

Our interdisciplinary hospice team provides a personalized plan of care focused on pain and symptom management, while emphasizing quality of life and comfort. Hospice services available as needed may include:

  • Hospice care wherever you call home, including visits by a nurse. Nurses are also available 24 hours a day, 7 days a week to visit if you need help.
  • Medical supplies and equipment
  • Medication for pain and symptom relief
  • Home health aide services
  • Physical, occupational or speech therapy if needed
  • Dietary counseling
  • Spiritual guidance if desired
  • Support from trained volunteers
  • Short-term respite care
  • Temporary hospitalization if needed to help manage symptoms that cannot be controlled at home
  • Counseling and bereavement services to provide you and your family psychological, emotional and spiritual support

Hospice team

The interdisciplinary hospice team coordinates efforts on behalf of the patient and family, and each team member provides support based on his or her area of expertise. Working closely with the patient and family, our hospice team develops a plan of care tailored to each patient’s needs and situation.

  • Medical director: oversees each patient’s plan of care, advises the hospice team, and is available 24 hours a day, seven days a week
  • Registered nurse: visits the patient at home regularly to provide nursing care, support, teaching and instructions to caregivers
  • Hospice aide: visits the patient at home regularly to help with personal care, such as assistance with bathing, dressing and eating
  • Medical social worker: helps the patient and family cope with emotional, social, physical and financial stress, as well as provides counseling as needed
  • Spiritual care coordinator: provides spiritual support to the patient if desired, and serves as a liaison between patient/family and local clergy of their choice
  • Volunteers: specially trained community members who provide non-medical support and assistance to patients and their families including respite care, companionship and help with errands
  • Registered dietitian: consults with the hospice team, the patient and the family concerning nutritional needs and challenges the patient may be facing
  • Bereavement coordinator: arranges ongoing bereavement services, support and education for grieving family and friends
