Continuing Care from Hospital to Home

Conveniently located onsite at McLaren Lapeer Region, our Transitional Care Unit provides skilled nursing care and therapy services to residents who need a continuum of care from their hospital stay. Residents in the 19-bed Transitional Care Unit have access to hospital-based care and specialty services, but the setting is much more relaxed. Our multidisciplinary care team works closely with each resident and family to develop a personalized treatment plan. Together, our team of medical professionals will create an individual plan of care centered around our patient’s return to independence.

Services Available On-site

• Physical Therapy - Therapists work with residents and their families to design specific treatment programs based on present functional levels and agreed-upon goals.
• Speech Therapy - Speech-language pathologists evaluate and treat residents with voice, articulation and language disorders, as well as cognitive disorders and swallowing dysfunction.
• Occupational Therapy - Occupational therapists evaluate residents and design individualized plans of care for residents who have difficulty performing everyday living activities.
• Social Services - Our social worker helps residents with admission and discharge planning as well as referrals to support groups, educational programs and follow-up care.
• Wound Management - When needed, wound care specialists are available to provide care and education to residents with ostomies and wounds
• Respiratory Therapy - Oxygen therapy and breathing treatments are available from McLaren Lapeer Region's cardiopulmonary department.
• Dietary Services - A registered dietitian evaluates the needs of each resident and makes appropriate dietary recommendations. Individualized diet education is provided based on resident needs.
• Additional specialty services including critical care, emergency medicine, diagnostic imaging, lab services and pharmacy.

What To Expect

The Transitional Care Unit at McLaren Lapeer Region is designed to help you recover from your hospital stay and prepares you to go home. Our interdisciplinary staff will help give you the support and guidance you need to reduce the probability of returning to the hospital.

Arrival

When you arrive to your room, our staff will assist you in getting settled into your new temporary living space. Within the first day, our interdisciplinary team will evaluate your needs and set your individual goals and treatment plan, all with the vision of you returning to your home in a couple of weeks.

While You Are at the Transitional Care Unit

During your stay with us, our team will continue to monitor your progress and work with our physicians to ensure you are meeting your goals and have an effective treatment plan. You can expect to receive an array of therapies in our therapy gym, while also participating in a variety of fun activities that are available on the unit. We also have multiple specialty services if any needs arise such as wound care management, pain management and laboratory or diagnostic imaging.

Discharge

Before you go home, our team will assure you have reached all of your goals to complete your daily tasks and return home safely.
