McLaren Physician Partners Launches MyCare

 Starting this week, McLaren Physician Partners (MPP) has launched a deviceless remote patient monitoring program called MyCare. MyCare sends condition-specific text messages or automated phone calls to McLaren patients to support care coordination and monitor their health virtually.


The goals of this partnership are to:
• Reduce medical costs by reducing ED visits, admissions and re-admissions
• Scaling care coordination services to engage a larger population
• Increase success with value-based care models


MyCare, an extension of our Care Coordination services, will help McLaren physicians stay connected to patients in-between scheduled appointments by monitoring patient-reported data regarding their symptoms for the following conditions:
• COPD
• Heart Failure
• Post Discharge


These messages collect patient reported information and alert the MPP Care Coordination team when a patient responds negatively which allows our care team to intervene before the patient's condition worsens to help manage their health and wellbeing. This program is offered to patients at no cost to them. Your office may receive a call from your patients about MyCare in the next few weeks. Patients are more likely to enroll if their provider encourages and supports MyCare.
