Specialist Collaborative Survey
As McLaren Physician Partners (MPP) enters at-risk contracting, it is important to continue to build relationships within our Specialist network. We developed a Specialist Collaborative Survey to identify best practices for communication and collaboration between specialists and primary care providers in population health management and performance-based models of care.
The survey questions highlight specific communication and operational activities which are important to population health management and providing the best care for your shared patients. Your responses will help MPP continue to improve our network of high performing specialists.
The benefits of inclusion in the high performing specialist network will include marketing to the MPP primary care referral base.
