Please bring the following items on the day of the patient’s admission:

  • Clothing and Personal Items
  • Social Security Card, Medicare Card, and other Insurance Cards, if applicable
  • Advance Directives
    (i.e., Living Will, Durable Power of Attorney), if applicable
