Diabetes Self-Management Education Program
Through group classes and an individual assessment, patients can gain the knowledge, support and technical skills they need to successfully manage their diabetes. They also learn to develop a healthy lifestyle through proper nutrition, medication, exercise and home blood sugar checks.
The education program helps adults with Type I, Type 2 or gestational diabetes who are seeking to improve diabetes management, whether they are newly diagnosed or have had diabetes for a long time.
Class topics cover the most current information available to help patients learn how to manage their blood glucose levels. Topics include monitoring and managing your blood glucose; medication management; reducing your risk of complications; carbohydrate counting; how to balance meals; meal planning; sick day management; exercising safely and more.
This program is recognized by the American Diabetes Association for meeting the National Quality Standards for Diabetes Self-Management Education and is certified by the State of Michigan Department of Health and Human Services.
The program is covered by most insurance companies. Our team of registered dietitians can help navigate patients through the insurance process.
