Industrial Health Information: 

• Clinic Hours:  8 am to 5 pm, Monday through Friday.
• Clinic address: 1644 Stone Street, Port Huron, MI 48060. Map available.
• Last patient is seen at 4:30 pm to allow for adequate medical treatment before we close.
• Please send an Authorization Form in with each employee.  Authorization forms can be printed from this website.
• Call the clinic -- (810) 982-8016 -- before sending an employee in for injury or illness treatment.
• Appointments must be made for all physical exams.
• Drug screen only visits are on a walk-in basis.  No call is needed if employee has an Authorization Form.
• When the clinic is closed, please direct all work related injury/illness care to the Emergency Center at McLaren Port Huron hospital -- 1221 Pine Grove Avenue, Port Huron, MI 48060.  Please send the employee to the Emergency Center with an Authorization Form. 
• Cell Phones and Pagers must be shut off inside the clinic.
• Please bring proof of identification for any testing procedures.

Industrial Health Information For Employers:

• Appointments must be made for all physical exams.  Drug screen only visits are on a walk-in basis as long as employee has an Authorization Form.
• Please call the clinic before sending an employee in for a work related injury or illness.
• McLaren Port Huron Industrial Health bills only workers compensation.  No personal insurance is accepted.
• If McLaren Port Huron Industrial Health deems an injury or illness"not work related", the employer is responsible for all expenses rendered up until the time of such determination.  McLaren Port Huron Industrial Health does not bill patients or any personal health insurance.
• All work-related injury or illness cases must be reported via a first report by the employer to their respective workers compensation insurance carrier.  In cases where injury treatment is being paid directly by the employer, payment for services is expected within 30 days.
• W-9 forms for McLaren Port Huron Industrial Health are available to you to set up Industrial Health as a vendor.
• Payment terms are net 30 days.  If you are unable to meet the payment terms, it is your responsibility to contact McLaren Port Huron Industrial Health and make other payment arrangements.
• Any changes in services -- additions, subtractions or deletions of services -- must be requested in writing.  Requests can be emailed, faxed or sent via U.S. Mail.

• A valid photo ID is required for all services.
• Patients walking in for services after 4:30 pm may be rescheduled or redirected to the McLaren Port Huron Emergency Center.
• Any appointment cancellations must be done through the employer.
• Please arrive on time for your appointment.  Patients that arrive late may be rescheduled.

Physical Therapy On-site Services Available

Industrial Health's Physical Therapist, Curt Jevens, is available to help you get a handle on any ergonomic issues that you may be having at your workplace.  Curt is available to perform:

• Ergonomic Screening -- A walk through your facility is performed to look at general concerns regarding ergonomics and work station set up to determine possible risks for onset of musculoskeletal disorders. 
• Ergonomic Analysis -- An in depth ergonomic analysis of a specific job is performed to identify risk factors for musculoskeletal disorders and offer specific solutions to address the risks that are present.
• Job Analysis -- A break down of a job into specific tasks is performed to develop a detailed job analysis.
• Back School -- Back injury prevention training is provided with emphasis on proper spine positioning during material handling tasks.  Anatomy of the spine is also reviewed and job specific lifting situations can be addressed. 

Working with Curt can help prevent costly injuries before they happen.  Many Industrial Health clients have had Curt on-site for services and have been extremely happy with the result.

Health Coaching Available

McLaren Port Huron's Industrial Health Physical Therapist, Curt Jevens, is available to provide Health Coaching services for employees who are interested in improving their overall health.  Health Coaches work with employees on a one-to-one basis to look at ways to improve health and prevent major health events.   The Health Coach helps the employee modify their lifestyle and hopefully make improvements to their health and well being through a planned program.

The Health Coach will provide the participating employee with health and diet information, paperwork they can use to document their progress, an appropriate exercise plan, diet planning as well as serving as a motivator helping the employee reach their goal. 

Health Coaching services are available to individuals at Industrial Health or to larger groups on-site.

Occupational Health Respiratory Protection Program

McLaren Port Huron Industrial Health can help you with your entire workplace respiratory protection program. Our team of experts can help you make sure your program is up to date and in compliance. We have all the equipment needed to properly fit test your employees and make sure they are safe from any respiratory hazards.
