McLaren Physician Partners (MPP) is pleased to announce the implementation of our population health services care management team consisting of patient navigators, nurses, and social work. Our goal is to work collaboratively with the practices to achieve an optimal level of wellness and improve coordination of care while providing cost effective, non-duplicative service to our patients.
MPP has engaged Athenahealth as our electronic population health management partner. The system will host 1) claims-based/utilization patient data, 2) admission, discharge and transfer activity (ADT), and 3) a care management module. This module will be used for care plan development, documentation and communication. Through analytics we will identify actionable-data and develop subsequent improvement strategies. Examples include high-risk populations, ED utilization and chronic condition management.
As of today, we have data on 90,000 beneficiaries. In the near future, you will receive communication regarding targeted interventions for your patients starting with transition of care management program. The care management team will communicate with your practice regularly through email, fax blasts, meetings and web-based in-services with the intent of keeping you fully informed of initiatives.
