Harbor Health Plan has requested to terminate its existing relationship with McLaren Physician Partners/McLaren Health Care System effective November 15, 2017.
The contract with Harbor Health Plan was for the following counties: Macomb, Oakland and Wayne. This includes our primary care physicians, specialists and three facilities at Karmanos, Macomb and Oakland locations.
After the termination date, any hospital services rendered to Harbor Health Plan members are to follow the Michigan Medicaid Access Agreement, physicians will be non-par and reimbursed Medicaid rates.
