Pharmacist Led Medication Reconciliation

 

Studies have shown that approximately 20% of hospital readmissions may be medication-related and 70% of these readmissions are possibly preventable.  McLaren Physician Partners (MPP) partnered with Outcomes Medication Therapy Management (MTM) and Walgreen’s to provide patients with a medication reconciliation by a pharmacist.  Starting next week, as part of our current Transition of Care services, high risk patients discharged from McLaren Macomb will be offered this service.

 

Primary Care Providers will receive documentation of the medication reconciliation either within Cerner or by Fax, if not on Cerner.  Pharmacists will not modify medication lists or orders.  If there are any discrepancies or subtherapeutic doses identified, the Primary Care Provider will be contacted. This program is offered to patients at no cost to them or their insurance company.  Patients will receive a written list by mail of their medications and detailed instructions.

 

The goals of this partnership are to:

  • Decrease medication discrepancies that occur during transitions of care from the hospital to home
  • Reduce avoidable medical costs by reducing ED visits, admissions and re-admissions
  • Provide the most accurate medication list for providers to use during the transition of care visit
  • Increase patient knowledge about their medications and improve adherence to medication regimens

This program is an extension of our Care Coordination services and was developed to continue to improve the quality of care provided to our patients.  Patient and Provider surveys will also be distributed to solicit feedback and measure satisfaction with the program.
