Obtaining Your Medical Records

A complete inpatient, outpatient surgery or emergency record of each patient's care, treatment and progress is kept in the Medical Records Department. You have the right to expect that medical information regarding your care will be treated as confidential, and

McLaren Port Huron has the obligation to safeguard your records against unauthorized disclosure.

Release of your medical records

Medical records may only be requested by the person they belong to or their legal representative according to HIPAA Federal laws.

Driver's License or Picture ID MUST be presented.
If you are sending someone on your behalf to pick up your records, you will need to make mention of this person's name in the area on the Authorization for Release of Information and make sure to sign the Authorization for Release of Information giving McLaren Port Huron permission to release your records to this person . The designated person must bring photo identification in order for them to be released. This person can be the patient, legal guardian, patient advocate, or personal representative.

Medical Record Copying Services:
McLaren Port Huron has contracted HEALTHPORT to do its copying of medical records. This national copy company is fully insured and licensed to handle confidential information. Any questions about the status of your request can be directed to the HEALTHPORT REPRESENTATIVE at (810) 985-2694. Please leave a voice mail message if the Copy Representative does not answer. Someone will return your call.

Fill out the Authorization to Release Medical Records form. Be sure to include the specific dates of service needed, specific type of information along with the reason for the request. The form can be obtained at:

My McLaren Chart Online Portal

Access your medical record anytime, anywhere

My McLaren Chart provides patients with electronic access to their health information, anytime from any location. The information is completely secure and comes directly from your electronic health record at McLaren Port Huron. Recent visit information, lab and radiology results, medications, allergies and care instructions are kept for you in your own personal electronic chart.

My McLaren Chart will allow you to review or print information using your desktop computer, tablet or smart phone. It will be always up-to-date and accessible whenever you need the information - 24/7. Printing information must be done from a desktop computer. At this time we are not able to reschedule or cancel appointments via our portal. For Surgery and OR scheduling, please contact your physician's office directly. For MRI scheduling, call us at 810-989-3292. For all other outpatient scheduling, call us at 810-989-3720.

To review information in My McLaren Chart:

My McLaren Chart will allow you to review information using the following supported browsers on your desktop computer, your tablet or smart phone:

    Desktop Browsers

  • Chrome (latest version; Firefox, latest version; Internet Explorer, version 11+; Safari Version 6+
  • Google Chrome and Mozilla Firefox automatically update to the latest released version. If a problem is observed with one of these browsers, make sure auto-updating has not been disabled and that the current version is in use.
  • Safari for Windows has not been updated beyond version 5, so this browser is no longer supported or Windows users.
  • Mobile Browsers

  • Apple IOS, Safari on IOS version 6+; Android, Latest version of Chrome; Microsoft Surface, Internet Explorer 11 on Windows 8+
  • Support for Apple ios is specified in terms of OS version. Safari is not independently updated outside of an OS version for a mobile device
  • Only Safari is supported on IOS. Chrome and other browsers available through Apple’s App Store are not supported.

To print from My McLaren Chart:

My McLaren Chart may be printed from your desktop computer using the latest version of the Chrome browser. Printing from other devices is not supported at this time.

To enroll, you must have provided your email address during the hospital registration process or during your hospital visit or stay. If you are uncertain whether you provided your email address, please call 810-989-3127 Monday-Friday, 8 a.m. to 4 p.m., or email us at contactus@porthuronhospital.org. Please include your name and telephone number. Please allow three business days for an email response.

If you did not share your email address, please call the Release of Health Information desk at 810-989-3127. Once your email address is entered into our system, you may click on My McLaren Chart.
