To submit a new activity request, please fill out the Preliminary Application. Download and save this application to your computer and submit to mclarencme@mclaren.org. The IPCE Committee will review the application and respond to you within 7-10 business days.

Once the preliminary application has been submitted and approved you will be contacted by the IPCE department to fill out the New Activity Application. Download and save this application to your computer and submit to mclarencme@mclaren.org. The IPCE coordinator will review the application and respond to you with any questions or suggested changes. Conflict of Interest Disclosure Form
Download the conflict of interest disclosure form and complete for all participants involved in the content of the activity (planner, speaker, teacher, reviewer moderator, author, etc).
