McLaren Bay Medical Foundation (Tax ID#: 38-2156534) is a nonprofit, supporting organization created to receive and administer funds to support the mission of McLaren Bay Region and McLaren Bay Special Care. All contributions directly benefit the donor designated purpose and stay local.

About McLaren Bay Region

McLaren Bay Region is a 415-bed acute care hospital providing specialty, primary and preventative care located in Bay City, Michigan. Services include cardiovascular, neuroscience, women’s health, orthopedics, rehabilitation, home health and hospice care, EMS and oncology. The emergency department treats more than 45,000 patients a year.

McLaren Bay Region is affiliated with more than 50 educational institutions for training a wide variety of clinical personnel. As the regional leader in cardiovascular and neuroscience services, McLaren Bay Region’s employees provide quality care for patients throughout the Great Lakes Bay Region with a population of nearly 500,000.

Sample Bequest Language

For balance or residue of estate after arrangements are made for family:

“After fulfilling all other provisions, I hereby give, devise and bequeath to McLaren Bay Medical Foundation, a nonprofit organization located at 1900 Columbus Ave., Bay City, MI 48708, Federal Tax ID #38-2156534, ALL OR A PERCENTAGE of the rest, residue and remainder of my estate to be used for the area of greatest need at the time the gift is received.”

For percentage of estate:

“I hereby give, devise and bequeath ____ percent (___%) of my total estate, determined as of the date of my death, to McLaren Bay Medical Foundation, a nonprofit organization located at 1900 Columbus Ave., Bay City, MI 48708, Federal Tax ID #38-2156534, to be used for the area of greatest need at the time the gift is received.”

For specific dollar amount:

“I hereby give, devise and bequeath _________ and No/100 dollars ($DOLLARS) to McLaren Bay Medical Foundation, a nonprofit organization located at 1900 Columbus Ave., Bay City, MI 48708, Federal Tax ID #38-2156534, to be used for the area of greatest need at the time the gift is received.”

For a contingent commitment gift:

“If (primary beneficiary) does not survive me, then I hereby give, devise and bequeath to McLaren Bay Medical Foundation, a nonprofit organization located at 1900 Columbus Ave., Bay City, MI 48708, Federal Tax ID #38-2156534, DESCRIPTION OF PROPERTY to be used for the area of greatest need at the time the gift is received.”
